I was thinking about it recently while working on mod_cache.

Realistically we can only really cache stuff if it has a predetermined content length. This way the cache modules can decide whether the object is small enough to cache. Objects without a content length are usually CGI scripts and streaming content, so I doubt this limitation will make much difference to cache efficiency.
