Such has been reported before. Sadly I don't known what it caused. (So I even don't known any workarounds for it ... )

Thank you for your feedback.

I have neither a definitive answer for the remaining issues.

For the disk renaming issue, I hope for some follow-up on this bugreport or on the mailinglist debian-boot.

For the BIOS Bug, I would go for a BIOS upgrade.

For the 'PCI: Not using MMCONFIG', I did a short websearch, what I understood from it, is it related to the BIOS bug.
