I am also using htb3, and have not experienced problems with directly outgoing stuff.

I did have vanishing packet problems when the gateway was routing from one tunnel to another tunnel (between a wireless laptop tunnel and a remote subnet). And when I added imq, that caused kernel panics and complete freezes, but playing with iptable rules I managed to get it working nicely and reliably.
