[patch 0/1] For Review: WL4092 remove ndb_use_exact_count

Difference from last patch:
- runs util thread even when query cache is disabled (to keep row count
cache up to date).
- all tests pass (results updated)

ported to 5.1-telco

I propose this for current -telco ..... maybe even 6.3. But would be nice to get this into MySQL 5.2 (6.2?). Thoughts welcome.
