The reason the milter stuff is not built by default is due to the fact that pthreads does not work well on all platforms. You'll note that the milter bits in the Makefile say "untested" and that's the truth. I've just committed changes to the Makefile bits that should give things a better chance of working but please be aware that building sendmail in-tree with milter is not supported in any way at this time.
