I assume you mean you did something like:

    grep -Ri blacklist /*

I'm not familiar with spamass-milter - is there any way to tell it to call SA with debugging enabled and logged somewhere so that we can see for sure what it's doing?

Do you have the option of calling SA via a different path, e.g. procmail? That might be useful as a troubleshooting step just to see whether or not the behavior changes.
